Oak Park Place - Madison

Senior Living in Madison, WI just got better ... Oak Park Place’s newest community opened June 1, 2006.  The beautiful new site is located on Madison’s growing east-side ... just off Cottage Grove Road and I-90 at 718 Jupiter Drive, Madison, WI 53718.

It’s all brand new, part of Grandview Commons and anchors the west end of a multi-family (single family, duplex, apartments) residential neighborhood.  Oak Park at Grandview Commons features the full continuum of care and exemplifies the organization’s commitment to providing Madison’s seniors the opportunity to age in place.


Skilled Nursing:  a 100-bed skilled nursing facility with programming for seniors with dementia, or requiring skilled nursing assistance leads the community.  Rehabilitation services (post-op and ortho) are also available in the skilled nursing area.


Assisted Living: 64 beautiful apartment suites immediately adjacent to the Skilled Nursing section are available for rent. Ten floor plans allow seniors to pick an apartment (studio, 1 or 2-bedrooms) that matches their need for space.  All Assisted Living apartments are furnished with refrigerators and microwaves, cable TV, local phone, in-suite temperature controls and other amenities.


Memory Care in Assisted Living is part of Autumn Lane on the first floor of this beautifully appointed building with thirty-two apartments dedicated to programming for three levels of dementia care.  Prices for Memory Care apartments reflect the level of care required per resident. 

Memory Care at Oak Park Place is also renowned for its excellent programming and levels of care.  Autumn Lane refers to the Memory Care areas of both the Skilled Nursing and Assisted Living portions of the community.  Residents are treated with dignity and respect in all aspects of their lives on Autumn Lane.  The nursing staff, care-givers, housekeeping and programming staff is selected based on their compassion for seniors who suffer from dementia or Alzheimer’s.

Programming and activities are geared to the individual - it is not a cookie-cutter approach to dealing with Oak Park Residents.  This targeted approach to dementia and/or Alzheimer’s allows residents to follow a customized plan tailored to their needs.  Skilled professionals assess the levels of care required by each resident and a resident-centered plan of action is offered for each person.  This plan involves family, friends and the resident to construct the best course of action to allow the resident to remain active, productive and capable of aging in place – with dignity. Care-givers become familiar with the activity plans and gently encourage the resident’s participation.  Since the plan is constructed based on the input from those closest to the resident it’s more likely to succeed.

Independent Living - the Arbors: 81 luxury apartments are the newest addition to Oak Park Place at Grandview Commons. Spacious single-level, senior-friendly residences - one and two-bedroom units are available for lease.  All include stackable washer/dryers, low-nap carpeting, easy-grasp drawer and door handles, patios or balconies, housekeeping and underground parking.  Some units feature gas fireplaces.


Rehabilitative Services:  residents of Madison’s east-side will appreciate the convenience of Oak Park’s rehab services for their post-op, reconstruction rehab services.  All the services of downtown facilities, but without the driving and parking hassles, are available in the brand new Oak Park Place with its spacious rehab suites, pristine equipment and furnishings.


Main Street Mall
All residents, families, friends and visitors of Oak Park Place at Grandview Commons are invited to use the shops of Main Street Mall – the connection between all arms of the campus.  Main Street Mall hosts a beauty salon/barber, bank, pharmacy, game room, library with computer lounge and general store which will offer ice cream, sundries, gourmet coffee and café selections.
