Oak Park Place offers a 100 bed skilled nursing facility providing the Madison community with nursing care in a peaceful home-like environment.   We offer complete skilled nursing including specialized programming for seniors with Alzheimer's and Dementia.  We work with family and physicians to insure the best possible individualized care for your loved one.

We offer individualized care plans, social services, events and therapeutic activities to encourage mental and physical stimulation.

The Oak Park® Madison Rehabilitation Center:  Our rehabilitation center offers a variety of therapies and rehabilitation services including  post-op and ortho care.  We work with physicians to create a rehabilitation plan tailored to you or your loved one's needs.  We offer all the services of downtown rehabilitation facilities, but without the driving and parking hassles. The Oak Park Place rehabilitation center offers spacious rehabilitation suites, pristine equipment and comfortable furnishings.

Our Madison skilled nursing home is located in the Grandview Commons community on Madison's East Side.  Our campus offers independent living retirement apartments, assisted living retirement apartments, skilled nursing and rehabilitation services.  We serve the greater Madison Wisconsin community including Sun Prairie, Middleton, Fitchburg, McFarland WI , Monona and Burke. We also serve Maple Bluff, Cottage Grove, Waunakee, Shorewood Hills, De Forest, Stoughton and Verona Wisconsin residents.
